| Public pressure is mounting for dramatic improvement in the efficiency, effectiveness and quality of health and human services in the United States. Spending continues to grow at a substantial rate, while evidence of serious deficiencies in the quality of care mounts. Government agencies, provider organizations, health plans and employers are developing and implementing a range of strategies to improve quality and control costs. The Care Management framework incorporates assessment, risk management, identification of resources, planning, service facilitation and coordination, education, environmental optimization, advocacy, monitoring and evaluation.
