| On February 17, 2009 the American Recovery and Reinvestment Act (ARRA) was signed into law. Of the total $767B in this stimulus package, approximately $69B was related to healthcare and approximately $19.2B was targeted specifically to drive adoption of electronic medical records. Also included in the legislation were significantly enhanced privacy and security requirements which build upon HIPAA. Under ARRA, the Medicare and Medicaid EHR Incentive Programs will provide incentive payments to eligible professionals, eligible hospitals and critical access hospitals (CAHs) as they adopt, implement, upgrade or demonstrate meaningful use of certified electronic health record (EHR) technology. Certified EHR technology provides assurance to purchasers and other users that an EHR system or module offers the necessary technological capability, functionality, and security to help them meet the meaningful use criteria. Certification also helps providers and patients be confident that the electronic health IT products and systems they use are secure, can maintain data confidentially, and can work with other systems to share information. Participation in the Medicare EHR Incentive Program began in 2011. Participation in the Medicaid EHR Incentive Program (voluntarily offered by individual states and territories) generally began in 2011. On March 23, 2010, the Patient Protection and Affordable Care Act (PPACA) was signed into law requirements, with requirements to develop Accountable Care Organizations (ACO). Providers organized as ACO that voluntarily meet quality thresholds can share in the cost savings they achieve for the Medicare program. To qualify as an ACO, organizations must agree to be accountable for the overall care of their Medicare beneficiaries, have adequate participation of primary care physicians, define processes to promote evidence-based medicine, report on quality and costs, and coordinate care. Electronic health records, personal health records and other technology will be key success factors for organizations to obtain compliance and optimize business opportunities under these health care reform provisions. Thus, efficient exchange of information to integrate complex clinical workflows with the business processes of health and human service organizations will spell success in the future. Service organizations should identify the information needs of all stakeholders, analyze how data is utilized, apply an effective planning process and implement systems with the active involvement of key constituencies.
