| Quality is doing the right thing for the right client, at the right time, and doing it well. It is the degree to which service is safe, effective, accessible, timely, coordinated, affordable and appropriate for the individual person. By maximizing the many dimensions of quality, optimal outcomes and satisfaction can be achieved for the client and the overall community served. Continuous quality improvement activities then build upon that established foundation. In addition to ongoing quality management activities, health and human service organizations face other complex regulatory and accreditation requirements from the Centers for Medicare and Medicaid Services (CMS), the Joint Commission on Accreditation of Healthcare Organizations (JCAHO), the HRSA Bureau of Primary Healthcare (BPHC) and others. Assertive consumers demand secure, convenient and timely access to their personal information. Extensive HIPAA and ARRA regulations touch nearly every system and process connected with service delivery and payment. On January 16, 2009, the Department of Health and Human Services (HHS) published two final rules to adopt updated HIPAA standards — one related to the 5010 transaction standard and one related to ICD-10 codes. HHS modified the standard medical data code sets for coding diagnoses and inpatient hospital procedures by concurrently adopting the International Classification of Diseases, 10th Revision, Clinical Modification (ICD-10-CM) for diagnosis coding and the International Classification of Diseases, 10th Revision, Procedural Coding System (ICD-10-PCS) for inpatient hospital procedure coding. The implementation date for ICD-10-CM and ICD-10-PCS for all covered entities is October 1, 2013. HIPAA version 5010 accommodates the ICD-10 code sets, and therefore has an earlier January 1, 2012 compliance date in order to ensure adequate testing time for the industry. On February 17, 2009 the American Recovery and Reinvestment Act (ARRA) was signed into law. Of the total $767B in this stimulus package, approximately $69B was related to healthcare and approximately $19.2B of that was targeted specifically to drive adoption of electronic medical records. Also included in the legislation were significantly enhanced privacy and security requirements which build upon HIPAA. They are deemed essential to instill public and industry confidence in protecting sensitive identifiable health information. While these mandates present opportunities to streamline business processes and achieve a competitive advantage, implementation efforts can be time consuming and disruptive.
